SURRENDERING A DOG IN THE UK

If you find yourself in the unfortunate position of needing to rehome a dog within the UK and

need our help please provide us with the following information in order for us to assist:

• Name of dog

• Sex of dog

• Breed

• Date of birth

• Microchip

• Vaccination record

• Date of last flea treatment

• Date last wormed

• Spayed/Neutered – yes/no

• Any medical conditions

• Any current treatment or medication

• Any behavioural issues – e.g. food aggression, reactivity to other dogs, separation anxity

• Good with children

• Good with other dogs

• Good with cats

• Name, address and telephone number of vet

• Permission for us to make contact with the vet

Unfortunately, without all of the above information it makes it very hard for us to help.

We need all of this detail before we can even consider helping rehome your precious pup.

Once we have the answers to ALL of the above and if we have agreed to help rehome your dog -

you will then be asked to complete a relinquishment form.
